***ALL CURRENT PATIENTS PLEASE READ***

*************************************************

Help our practice stay strong, vital and responsive by choosing a visit over a message for medical questions. For example if you need a LAB ORDER or HAVE MEDICAL QUESTIONS for Dr. Clark, please make an appointment via the portal. Review your last visit on the portal to figure out when your last physical.  Labwork and physicals can wait two year mark if patients have concerns about coming in because of COVID. Your questions will be answered in 24 hours so please only ask once. Forms questions will take longer.  Please do not email kc@wellbeingmedicalcare.com, this is a non working email.  You can schedule a virtual appointment or in-person.

*****************************************************************

ADMINISTRATIVE QUESTIONS:

Non-Medical Questions -

Email idealmedical@myupdox.com  Portal password reset, pharmacy changes, records release, etc. You should get a response in 1-2 work days.

If you want to pay a bill electronically, email tc@wellbeingmedicalcare.com and an invoice will be created through quickbooks and emailed to you. If you look through your emails for every bill he is emailing patients this option.  He does billing weekly so give him time to respond.

**************************************

FOR ACTIVE PATIENTS:

Fee for your virtual visit is copay and balance billing. Insurance will cover as a regular office visit. If an office visit is needed I will arrange it. I will continue to see people virtually prior to an office visit. Office visits can now include physicals as of May 7th, 2020. Patients can be seen for non-urgent requests.

STEPS TO SCHEDULE: After signing in to the portal, CLICK on the FORM tab above and complete 2 forms:

(1) Returning Patient Form - Before Any Physical Fill Out

(2) Returning Male or Female Do Medical History Questionnaire

Schedule a virtual visit where we will go over those and decide on lab work and then I will schedule your physical.  Telephone $25 visits are no longer available as all visits will be virtual visits. Also note that when wellness physical exams restart, most insurances want these to be billed as a dual visit (meaning you will owe a copay) if you have any chronic or acute issues that you want addressed at that visit. They will only cover it as a preventative code for you if there are no medical issues addressed, except for preventative issues such as discussing mammography, colonoscopy, lipid screening, etc.

I CAN OFFER BEFORE AND AFTER HOUR VIRTUAL VISITS; PLEASE ASK AND I WILL SCHEDULE.

CURRENT ACTIVE PATIENTS

Patients not seen in over 3 years automatically becomes inactive on our system. Please follow the directions for new patients by sending an email to idealmedical@myupdox.com to be put on the waitlist. We keep an active panel just enough to to be able to see patients same day/next day. If you are scheduling a physical then complete a medical questionnaire. If your insurance has changed, please fill out the insurance form and let us know before your appointment. Dr. Clark participates with Blue Cross Blue Shield (all types), Cigna, and Medicare as a primary insurance. Dr. Clark is an out-of-network provider with all other insurance plans. If we do not respond by portal within 24 hours or as quickly as needed, please call our office number 410-730-8288 and pick option 3 which connects you to Dr. Clark OR email kc@wellbeingmedicalcare.com.  Texting is not a reliable way to contact Dr. Clark since it does not get checked regularly.

PHONE AND VIRTUAL VISITS

Clinical care by phone or email is a non-covered service meaning your insurance will not cover. This does not pertain to urgent questions or calls, these of course are not charged. For cases where you choose phone visit as an option there is a fee of $25-45. Examples include: medications and or treatments for colds/congestion or urinary tract infections or GI illnesses. We can best accommodate your need if you use the scheduler and choose phone visit and list a contact phone number. Payment is by PayPal kc@wellbeingmedicalcare.com or check mailed to Well Being Medical Care. We cannot bill them by mail so we would appreciate payment at the time of service.

HOW TO ACCES DR. CLARK

Patients can reach Dr. Clark through portal message, email, text, phone call or by scheduling a visit.  This is a low volume slower paced visit based practice.  We do not counsel on conditions, prescribe over the counter or prescription medications or order lab work without a visit associated with medical advice or treatment. Below is a list of ways to reach Dr. Clark and when to expect a response.  Attempting to reach her more than one way will slow her response time.

OFFICE VISITS

Office visits are available same day to next day very often within hours of checking the portal.  Since it is automated patients do the scheduling 24/7.  Our office hours are listed on the portal.  If you know you want Dr. Clark to counsel about OR treat a medical condition, schedule an appointment. If your question is "Do I need to be seen?" the answer is always yes. Dr. Clark advise against asking if a visit is available or needed as this delays care. She advise going on the portal and schedule right away. Visits are available during weekdays, one evening a week with few exceptions and early morning Thursdays. If there is an urgent sick visit and you want to see Dr. Clark and there are no visits, please message her a request as she comes back in the evening if needed. Phone visit explanation is above.

PORTAL MESSAGE

Secure way to communicate with Dr. Clark.  Patients can send messages at any time 24/7.  Response time during our business hours is fast.  The longest within 12 hours (or 24 hours if sent on late Saturday or Sunday).

EMAIL

Emails are not secure so only recommended for administrative questions with response time of 1- 2 work days.

PHONE

Response time for phone calls is the slowest as Dr. Clark cannot get to these until after office hours are completed. The Phone system is automated.  Phone calls can be made to Dr. Clark 24/7 and she answers urgent calls 24/7. Call 410-730-8288 and option 1 gives general information. Option 2 leaves a message that is converted to text and sent to her email.  Option 3 rings her cell phone. Please use this for calls where you are worried, concerned and/or want an answer right away.  It is not about if a medical person would think it is urgent.  The urgency based on your perception is what she cares about and want to respond to.

EMERGENCIES

There is always an option to call 911 so please do for medical emergencies and then at some point send a message.  If you have time to call Dr. Clark then use Option 3 which rings her cell phone.  Please use this for calls where you are worried, concerned and/or want an answer right away.  Dr. Clark does not judge if she thinks it is urgent that is up to you.

EMERGENCY ROOM or URGENT CARE VISITS

When an emergency, do what you need to do and as instructed above.  If you are going to be seen for illness outside Dr. Clark's office hours, consider notifying her in case she could make time to see you with the exception of Sundays where I have family commitments.  If you could time your visits outside of Sundays that would help as she is trying to increase her visits.

COMMUNICATION OF RESULTS

Dr. Clark provides information and services (referrals, appointment (as above), refills, test results, communication, forms) to patients through a secure electronic system the portal.  Requests can be made 24/7.  Response time for referrals, forms and lab results can take up to 5 days especially if Dr. Clark is away.  This is the longest it can take Dr. Clark will work on faster by request. mail and invitation are only intended for use by the named addressee. If you are not the intended recipient of this email or received this in error, please immediately and permanently delete this email. Note: Our practice uses email, text, and/or calls to notify patients of information available regarding care, test results, appointments, and financial statements.
