We do not answer the phone at the clinic, and handle the majority of our communication electronically. When we receive a voicemail it is sent directly to our e-mail account. Please leave us a detailed message, and we will get back to you within 24-48 hours.
Please call your pharmacy to verify that you do not have a prescription saved to your account if you have been seen within the past 3 months, before requesting a refill. (You may not refill controlled substances through automated pharmacy systems, and you will need to speak with a pharmacist to fill your medication.
NEW PATIENT INSTRUCTIONS
#1) Please complete the sign-up process and register for our patient portal at https://www.myupdox.com/portal/vfc/html/register.html
#2) Please complete new patient forms #1, #2, #3, #4, #5 and #6 that can be found under the "forms" tab of the portal.
#3) Please e-mail us at firstname.lastname@example.org and let us know what your appointment availability is - we will get back to you within 24-48 hours.
#4) Please bring your ID and insurance card to your first appointment.
New patient appointments generally run 1 hour in length.
