You are now able to schedule your own follow-up appointments through our patient portal.
We keep early and late appointments offline - please e-mail us if you would like to request an appointment after work or early in the morning.
Please complete your "Adult follow-up form" before the visit, located under the "forms" tab of the portal.
1. If you are interested in becoming a new patient with Kristina, we ask that you please register for our patient portal.
2. Please fill out the new patient forms #1, #2, #3, #4, #5 and #6 that can be found under the "forms" tab of the portal.
3. After completing your forms, please send us an e-mail at firstname.lastname@example.org and let us know what days/times are best for you.
- Please bring your ID and insurance card to your first appointment.
New patient appointments generally run 1 hour in length.
Pay your bill
We offer 20% off to those that pay within 24 hours of their appointment - please make your payment for the full amount if that window of time has passed. Thank you!
