We require previous medical records prior to your office visit so please go to http://www.shfmed.com/forms to print off and complete the medical release form. This must be printed off and signed, electronic signatures are not accepted. Once you have completed the form send it back to our office through a portal message or fax, 303-680-9149.
It can take several weeks for medical record requests to be processed. Please take this into consideration and complete this form as soon as possible to allow for the best continuity of care.
