Sandra Pike FNP-C will be transferring her practice to the office of Dr. Carlos Dator Jr. effective date February 28, 2026.
Dr. Dator Jr's office address is 177 West 4th St, Oswego NY, 13126, phone 315-343-2151.
We have mailed a letter to each patient regarding our closure which includes a release form to sign to transfer your records. If you did not receive this letter, please contact our office. Please return the release form to our office at your earliest convenience. Even if you are following Sandy to her new location, the NYS Health Department requires a signed records release to transfer your records to her new location.
Dr. Dator Jr. has agreed to be the custodian of our medical records. After February 27, 2026, your medical records will be available from Dr. Carlos Dator Jr's office.
It is very important for you to follow up with a new provider for ongoing medical care. You will need to contact your new provider for any prescription refills after our office closes.
If you have any lab tests, imaging studies or visits with a consultant after our office closes, please let them know who your new provider is so the results and reports can be sent to your new provider.
