Please use SUBJECT LINES for the following categories when sending a message to us and include the following info:
1. Refill - include drug name, dose (milligrams, etc ordered) and directions on bottle. ALSO include what pharmacy you want to send it to. ****THEN check with your pharmacy within 2 business days.
2. Appointment - include your symptoms and severity. If you feel it is URGENT, then you need to call the office directly at 337-439-1484 to schedule a "sick today" appointment. If you feel this is an EMERGENCY - DO NOT USE THIS PORTAL. CALL 911
3. Questions - feel free to pose any questions you have and appropriate subject headings.

We are pleased to have a partnership with WCCH and WCCH Diagnostic Center. We can provide ON SITE Cat Scans, Ultra Sounds, Labs, and Xray!! Just bring in your orders from your doctor and we will be pleased to provide this service for you. NO APPOINTMENT NEEDED!
The Family Care Center of SWLA is an independent health care provider. We are affiliated with WCCH, Cornerstone Hospital, St. Pats, and LCMH. We are committed to QUALITY healthcare of Westlake and surrounding communities and are here to stay. We have recently achieved a level three Virtual PCMH status and are proud to be a Quality Blue Provider.
Hospitals: We have working relationships with hospitalists serving WCCH, LCMH, and St. Pats and work closely with all specialists in SWLA. We also have relationships with St. Jude Childrens Hospital and MD Anderson Cancer Centers and are happy to coordinate and collaborate your care with these providers.
