Choosing a healthcare provider is one of the most important decisions you can make! You should look for someone who is caring, personable, knowledgeable, and accessible. Someone willing to take the time to go over all of your options and treat you as a person, not just another patient. At our family and sports practice, we pride ourselves on our patient service. We offer a personable and knowledgeable staff, a caring environment, and a respected reputation.

We are a Level 3 Patient-Centered Medical Home (http://pcmh.ahrq.gov/page/defining-pcmh). The primary care medical home is accountable for meeting the large majority of each patient's physical and mental health care needs, including prevention and wellness, acute care, and chronic care. We provide primary health care that is relationship-based with an orientation toward the whole person. 

Partnering with patients and their families requires understanding and respecting each patient's unique needs, culture, values, and preferences. The medical home practice actively supports patients in learning to manage and organize their own care at the level the patient chooses. Recognizing that patients and families are core members of the care team, medical home practices ensure that they are fully informed partners in establishing care plans. 

We deliver accessible services with shorter waiting times for urgent needs, enhanced in-person hours, around-the-clock telephone or electronic access to a member of the care team, and alternative methods of communication such as email and telephone care. 

We coordinate care across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services and supports. 

Our primary care medical home demonstrates a commitment to quality and quality improvement by ongoing engagement in activities such as using evidence-based medicine and clinical decision-support tools to guide shared decision making with patients and families, engaging in performance measurement and improvement, measuring and responding to patient experiences and patient satisfaction, and practicing population health management. Sharing robust quality and safety data and improvement activities publicly is also an important marker of a system-level commitment to quality.

Payments and Automatic Payments
*Make a Payment button above: easy way to make a one-time payment on a balance or bill at Longs Peak Family Practice without logging into the portal. Please note that we have the capability to establish payment plans, set up recurring payments, and store credit cards if you log into the portal and use the Payment button. We encourage you to keep a credit card securely on file to cover your bills after your insurance company has covered their part. We will still send you a receipt so you are aware of all charges, but it prevents you from having to log in, come in, or send us a payment and it saves us time sending you repeat statements. Email Policies:
LPFP now has a secure way for you to communicate with the office within our patient portal.

You can email general questions or any of the listed topics:
-Update patient demographics
-Payment receipts
-Appointment confirmations
-Request copy of labs or imaging report
-Medication refills (please give the standard 3 business day notice)
-Change scheduled appointment (if appointment is over 24hrs in advance)
-Schedule appointments that are not urgent (e.g., physical, diabetes checks, etc.)
-Blood pressure or blood sugar logs (will have to manually enter them in email message)

We will NOT take any emails asking or sending the following information:
-Requesting a medical diagnosis
-Detailed medical advice requests
-Urgent appointments or appointments needed same day

Simply log into the Portal and press the "send message" button located at the top right of the page. If we have your email address you will automatically be sent an email with log in information. If you have a password and forget it you may request another. Unfortunately, there is no way to attach files in the email in a secure manner at this time. You may use the email forms@longspeakfamilypractice.com for information you choose to send non-securely or you may fax forms to 303-772-8207.
