2. Need Appointment

This system is for non-emergency appointment requests

If you have an emergent medical problem such as shortness of breath, difficulty of breathing, chest pain, or abdominal pain. Do not use this system to schedule an appointment. Call 911 and go immediately to the hospital.
If necessary you may give a brief explanation of your problem
The dates and times are subject to availability and will be finalized when the office staff calls you back.

Please have your insurance information available so this may be verified when the office staff calls you to schedule your appointment. Please allow 24 hours for the staff to call you back on normal week days. On holidays or weekends, the messages will be answered on the next business day. If this is an emergency, do not use this appointment request system, stop what your are doing, and call 911.

For example: Aetna, Humana, Blue Cross, or Self-pay.
You will find this on your insurance card. If your are self pay you may leave this space blank.
